No, you do not automatically need TMG with NMN — but it can be a sensible optional addition. NMN is a direct precursor to NAD⁺, the coenzyme that powers cellular energy, DNA repair and sirtuin activity, all of which decline with age. The concern that NMN uses up your body's methyl groups comes from what happens after NAD⁺ is used, not from NMN itself. In practice, published human trials of NMN have not shown a clinically meaningful rise in homocysteine (the main marker of methylation stress), whereas high doses of plain nicotinamide have.

What is NMN and how does it work?

Nicotinamide mononucleotide (NMN) is a molecule your body makes naturally and one of the most direct building blocks of nicotinamide adenine dinucleotide (NAD⁺). NAD⁺ is a coenzyme involved in hundreds of reactions — from turning food into cellular energy, to DNA repair, to switching on the sirtuin "longevity" enzymes. NAD⁺ levels fall substantially as we age, and topping up its precursors is the central logic behind modern longevity supplementation.

NMN and the NAD⁺ salvage pathway

Once absorbed, NMN is converted into NAD⁺ by an enzyme reaction that does not directly consume methyl groups. This is an important and often-missed point. When your cells then use NAD⁺, one of the by-products is nicotinamide (NAM). Your body recycles most of this NAM straight back into NAD⁺ through the salvage pathway. Only the excess nicotinamide is disposed of, and it is that disposal step — not NMN conversion — where a methyl group is spent.

Human data support the safety of NMN at the doses people actually use. What is TMG (trimethylglycine)?

TMG stands for trimethylglycine, also called betaine or betaine anhydrous. It is a naturally occurring compound first isolated from sugar beets, which is where the name "betaine" comes from. Chemically, it is a glycine molecule carrying three methyl groups — and those methyl groups are the whole point.

TMG as a methyl donor

Methylation is a fundamental housekeeping process: your cells constantly add methyl groups to DNA, proteins and other molecules to regulate how they behave. The universal methyl "currency" is S-adenosylmethionine (SAM). When SAM donates a methyl group it becomes S-adenosylhomocysteine (SAH), which is then broken down to homocysteine — an amino acid that is undesirable at high levels. TMG helps recycle that homocysteine back into methionine via the enzyme betaine-homocysteine methyltransferase (BHMT), effectively restocking the methyl pool.[7] This is exactly why TMG is proposed as a companion to NAD⁺ boosters.

Dietary sources of betaine

You already get betaine from food. The richest sources are beetroot, spinach, wheat bran and other whole grains, quinoa, and shellfish such as prawns.[12] A typical mixed diet supplies a few hundred milligrams of betaine per day, while supplements deliver gram-level amounts. If your diet is already rich in these foods, your baseline methyl-donor status is likely to be reasonable — one reason the "you must take TMG" advice is often overstated.

The methylation theory behind the pairing

The "take TMG with NMN" advice rests on a genuine piece of biochemistry, so it is worth understanding properly rather than dismissing it.

The role of the NNMT enzyme

When there is more nicotinamide than your cells can recycle, an enzyme called nicotinamide N-methyltransferase (NNMT) methylates it for disposal. NNMT uses SAM as the methyl donor, producing methylnicotinamide (MNA) plus SAH.[4] Because SAH is broken down to homocysteine, sustained high nicotinamide clearance could, in theory, nudge methyl demand up and homocysteine up. Reviews of NNMT biology describe this same pathway and note that heightened NNMT activity can, in principle, lower NAD⁺ and raise homocysteine over time.[5]

What happens to homocysteine

Homocysteine matters because elevated levels are associated with cardiovascular risk. If a supplement quietly pushed homocysteine up, that would be a reasonable concern — and it is the single strongest argument put forward for pairing a methyl donor like TMG with an NAD⁺ booster. The key question, then, is not whether the pathway exists (it does), but whether NMN at real-world doses actually stresses it enough to matter. That is where the human evidence becomes decisive.

What the human research actually shows

This is where a lot of online advice gets ahead of the science. The methylation theory is frequently presented as settled fact, but the human trials tell a more measured story.

NMN safety trials and methylation markers

Across multiple published human trials — spanning single doses up to 1,250 mg and daily dosing of 250 mg for 12 weeks — NMN has been consistently well tolerated, with mild, transient effects (occasional headache, flushing or minor digestive complaints) reported at similar rates to placebo. A 2023 review of NMN human clinical trials in Advances in Nutrition concluded that the safety profile to date is reassuring, while emphasising that long-term data remain limited.[9] Crucially, no NMN trial has attributed a clinically meaningful rise in homocysteine to NMN.

The nicotinamide-versus-NMN distinction (2026 head-to-head trial)

The most useful evidence yet comes from a 2026 randomised, placebo-controlled trial published in Nature Metabolism, which gave 65 healthy adults 1,000 mg per day of NMN, nicotinamide riboside (NR), plain nicotinamide (NAM) or placebo for 14 days and measured circulating NAD⁺, gut-microbial metabolites and homocysteine side by side.[1] Two findings stand out. First, NMN and NR sustainably raised NAD⁺ — roughly doubling it — while plain nicotinamide did not sustain the increase. Second, it was nicotinamide, not NMN or NR, that produced an acute rise in homocysteine, reflecting the extra methylation demand of clearing a large nicotinamide load.

Bottom line from the trial: the methyl-drain worry is most relevant to high nicotinamide exposure — not to NMN at typical supplement doses. This reframes the "you must take TMG with NMN" claim as cautious rather than compulsory.

Does TMG lower homocysteine?

TMG is a legitimately effective homocysteine-lowering nutrient — but mainly at higher doses than most people put in a longevity stack. Randomised trials in healthy adults found that 3 g and 6 g of betaine per day lowered plasma homocysteine (by roughly 10% and 14% respectively), while 1 g per day produced no significant change.[8] A separate trial combining low-dose B vitamins with 1 g of betaine reduced homocysteine in adults with elevated levels.[6] So the everyday 500 mg–1 g of TMG bundled with many NMN products is a modest methylation "top-up," not a powerful homocysteine treatment.

There is an important caveat: at the higher 6 g per day dose, betaine has been shown to raise LDL cholesterol and triglycerides in healthy people, which could offset the cardiovascular benefit of lowering homocysteine.[11] This is a good reason not to assume "more TMG is better." Who probably does not need extra TMG

If you are a generally healthy adult taking a standard NMN dose (say 250–500 mg per day), eat a varied diet with plenty of vegetables and whole grains, and have no known methylation issues, the evidence does not compel you to add TMG. Your salvage pathway recycles most nicotinamide, your diet supplies betaine, and your homocysteine is unlikely to move meaningfully on NMN alone.

Who might benefit from adding TMG

TMG becomes a more logical addition if any of the following apply: you take high daily doses of NMN or stack several NAD⁺ precursors; you have an MTHFR gene variant that impairs folate-based methylation; you have measured elevated homocysteine; you eat few betaine-rich foods; or you follow a strict plant-based diet lower in choline and betaine. The MTHFR question

People with MTHFR variants convert folate to its active form less efficiently, which can reduce methylation capacity. Because TMG feeds a folate-independent methylation route (the BHMT pathway), it can be particularly useful for this group. That said, MTHFR status is best confirmed with testing and discussed with a clinician rather than assumed — and folate and vitamin B12 status should be addressed first, since they are the primary levers for homocysteine.

The foundational levers are folate (or methylfolate), vitamin B12 and vitamin B6, which are the primary regulators of homocysteine. Choline and phosphatidylcholine also feed the betaine pathway.
