The science: why these three are combined

NAD+ declines in ageing tissue

A well-cited study measuring NAD+ in human skin biopsies from newborns to 77-year-olds found that NAD+ falls progressively with age, alongside rising markers of oxidative stress.8 Similar declines have been reported in the brain and other tissues. It is worth being precise here: a 2026 Nature Metabolism analysis found that whole-blood NAD+ stays relatively stable across age, which is why researchers distinguish between blood and tissue NAD+.11 The rationale for the stack rests on the tissue-level decline, not on blood measurements.

How NMN and resveratrol could work together

The pairing has an elegant logic: NMN supplies more of the NAD+ that sirtuins need as fuel, while resveratrol activates SIRT1 so it can use that fuel.6,7 In principle, boosting the substrate and the enzyme together could produce more than the sum of the parts. Honesty matters, though: no human randomised trial has yet tested NMN and resveratrol together against NMN alone, so this synergy remains a well-reasoned hypothesis supported mainly by cell and animal data.

Why TMG is added: the methylation angle

When your body clears excess nicotinamide, an enzyme called nicotinamide N-methyltransferase (NNMT) attaches a methyl group taken from the universal donor SAM (S-adenosylmethionine).9 The concern is that heavy NAD+ turnover could draw down the methyl pool. TMG helps top it back up by donating methyl groups, and it also has an independent, well-documented effect: at higher doses it lowers plasma homocysteine, a cardiovascular risk marker.4 Whether methyl depletion is actually a real-world problem at longevity NMN doses has not been confirmed in human trials — a point we return to below.

In clinical trials, NMN has generally been well tolerated with no serious adverse events reported over the short study durations tested.3 Mild digestive upset, nausea or headaches are the effects people most often mention across NAD+ precursors and resveratrol. A 2024 systematic review and meta-analysis of eight randomised trials found that short-term NMN did not significantly improve glucose or lipid markers in the general population, even though specific groups — such as prediabetic women in the Science trial — did show benefits.1,2 Resveratrol's ability to activate SIRT1 in people is also inconsistent across clinical studies, partly because of its poor bioavailability.6

The fair summary is a plausible mechanism with promising but incomplete human evidence.
