Three rules that matter more than the table: get sleep, movement and blood pressure right first; add one compound at a time and give it 8–12 weeks; and buy only third-party-tested products with a Certificate of Analysis. If you follow nothing else in this guide, follow those.

Do longevity supplements actually work?

The honest answer is: some do something, most are unproven, and none is a substitute for the basics. This is the most important sentence in the entire guide, and it is the opposite of what most supplement pages will tell you.

The single most robust finding in the whole longevity literature is not about a capsule at all. Sleep, regular movement, blood-pressure control, not smoking, and a Mediterranean-style diet still outperform every supplement ever tested for healthy lifespan. A longevity stack is a layer you add after that foundation is in place — never a replacement for it. If your sleep is broken and you are sedentary, no amount of NMN will move the needle.

Within supplements, the evidence divides sharply. A handful of compounds have genuine randomised controlled trial (RCT) data in humans showing measurable biological effects. A larger group has promising early human data or strong mechanistic rationale but no outcome trials. And a sizeable third group is marketed aggressively on the back of mouse studies alone. We separate these three tiers explicitly below, because conflating them is exactly how people end up spending £150 a month on hope.

Biomarkers versus how you feel

One of the hardest things for a new buyer to grasp is that a supplement can be "working" biochemically while you feel nothing at all — and, conversely, that feeling more energetic within days is more likely to be expectation than pharmacology. When a trial reports that NMN doubled blood NAD+, that is a real, measured change. But NAD+ is not a feeling; it is a coenzyme. The functional benefits that follow from raising it — better endurance, preserved muscle, slower biological ageing — accumulate over months and are subtle even when present. This is why the honest longevity claim is almost always about biomarkers and long-run risk rather than day-to-day wellbeing, and why any product promising you'll "feel 10 years younger in a week" is selling the placebo response rather than the compound.

The placebo effect is not trivial in this category, either. Longevity buyers are, almost by definition, health-motivated, hopeful, and paying attention to their bodies — all of which amplify perceived benefit. That is precisely why randomised, placebo-controlled trials matter so much here: they are the only way to separate what the molecule does from what wanting it to work does. Throughout this guide, when we say a compound is "Tier 1", we mean it has beaten placebo in a controlled human trial — not that users report liking it.

Healthspan, not lifespan — the goal that actually makes sense

It is worth stating the realistic aim plainly. No supplement is going to add decades to your life. What the better compounds may do is compress morbidity — shorten the frail, unwell period at the end of life — so that you spend more of your years mobile, sharp and independent. That is healthspan, and it is a goal supported by mechanism and, for a few compounds, by measurable functional endpoints like grip strength and walking distance. Framed that way, a sensible longevity stack stops looking like a bet on immortality and starts looking like what it is: risk reduction and functional preservation, bought at the margins, on top of a genuinely healthy life.

It is also worth being upfront about the compounds that have quietly failed. Several once-hyped ingredients have underwhelmed when finally tested properly in people. Editor's note — Dr Afzal: In my experience reviewing these products, the patients and readers who get the most from longevity supplements are the ones who treat them as a small optimisation on top of an already-good routine. The ones who are disappointed almost always bought a stack hoping it would compensate for poor sleep, no exercise, or an unmanaged blood-pressure reading. Fix the foundation first; the supplements make far more sense afterwards.

The science of ageing: what you're actually trying to slow

To judge any longevity supplement, you need a rough map of what ageing is at the cellular level — because a good supplement targets a specific, measurable driver of ageing, and a poor one targets nothing in particular.

The twelve hallmarks of ageing

The most influential framework in the field was set out by López-Otín and colleagues, who in 2013 defined nine "hallmarks of ageing" and in a landmark 2023 update in the journal Cell expanded these to twelve: genomic instability, telomere attrition, epigenetic alterations, loss of proteostasis, disabled autophagy, deregulated nutrient sensing, mitochondrial dysfunction, cellular senescence, stem cell exhaustion, altered intercellular communication, chronic inflammation, and dysbiosis of the gut microbiome.1 The crucial point is their third criterion: a true hallmark is one you can therapeutically slow, stop or reverse — which is precisely where supplements attempt to intervene.

  • Mitochondrial dysfunction. Mitochondria are the power plants inside your cells, and their capacity to generate energy declines with age. This underlies much of the fatigue, reduced exercise tolerance and slower recovery that people notice from their forties onward. Compounds like NMN (via NAD+), Urolithin A (via mitophagy) and CoQ10 (via the electron transport chain) all aim, in different ways, at this single hallmark — which is why they recur in almost every serious stack.
  • Deregulated nutrient sensing. Your cells have ancient molecular sensors — pathways such as mTOR, AMPK and the sirtuins — that respond to food, fasting and exercise. Keeping these appropriately balanced is one of the most conserved longevity mechanisms across species, and it's the pathway that caloric restriction, fasting and exercise act on directly. Several supplements claim to nudge these sensors, though few have proven they do so meaningfully in humans.
  • Disabled autophagy. Autophagy is your cells' recycling and quality-control system, clearing out damaged proteins and organelles. It slows with age, allowing cellular "junk" to accumulate. Fasting is the most reliable way to switch it on; spermidine and Urolithin A are the supplements most credibly linked to it.

If a supplement can't tell you which hallmark it targets and by what mechanism, treat that as a red flag. The reverse is a useful buying filter: the compounds worth your attention can all name their target.

Biological age versus chronological age

Your chronological age is fixed; your biological age — an estimate of how worn your cells actually are — is not. This is the metric the longevity field genuinely cares about, and it is increasingly measurable through epigenetic "clocks" and blood-based panels. Understanding it reframes the whole exercise: the goal is not to add decades to your lifespan on the back of a capsule, but to compress the period of ill-health at the end of life — to increase healthspan. How to read a supplement study yourself

The single most valuable skill in this category is the ability to look past a headline and judge the evidence behind it. Four questions will protect you from most of the marketing:

  • Was it in humans? An enormous share of longevity claims rest entirely on studies in mice, worms or cell cultures. These are how research begins, not how it ends. A finding in a petri dish tells you a compound is worth studying — nothing more.
  • Was it randomised and placebo-controlled? Without a control group taking a dummy pill, you cannot tell the compound's effect from the placebo response, regression to the mean, or the natural course of things. "Users reported improvements" is not evidence.
  • What was actually measured? A trial can be technically positive while measuring something trivial. Raising a biomarker is not the same as improving a health outcome. Look for functional endpoints — strength, walking distance, cardiovascular events — not just a molecule going up in a blood test.
  • Who funded it, and how big was it? A 20-person, manufacturer-funded study is a starting signal, not a verdict. Independent replication in larger samples is what turns a promising compound into a proven one.

Apply those four questions to any longevity product you're considering and you will immediately be a more discerning buyer than most of the market. They are also exactly the lens we use to assign the evidence tiers below.

The levers that beat every supplement

Two non-supplement interventions deserve special mention because they have stronger longevity evidence than almost any pill. The first is autophagy, the cellular clean-up process switched on by fasting and by certain compounds. The second is Zone 2 cardio — low-intensity aerobic training that directly improves mitochondrial density and function. The populations with the world's longest healthy lifespans, the so-called Blue Zones, achieve their results through diet, daily movement and social connection — not supplementation. Any honest longevity guide has to put these first.

The compounds, ranked by evidence tier

Here is the heart of the guide: every major longevity compound sorted by how strong its human evidence actually is. This is deliberately more conservative than most supplement rankings, because grading by hype rather than data is how the category earned its poor reputation.

Tier 1 — the compounds with real human trial data

NMN and NAD+ precursors. NAD+ is a coenzyme central to energy metabolism and DNA repair, and its levels fall with age. NMN (nicotinamide mononucleotide) is the most-studied precursor for raising it. Human RCTs have now shown that oral NMN reliably increases blood NAD+ and is well tolerated: a 2022 dose-ranging trial in 80 healthy middle-aged adults found that 300–900 mg daily for 60 days raised NAD+ dose-dependently and improved six-minute walking distance,2 and separate safety trials confirmed tolerability at doses up to 1,250 mg per day.3 The honest caveat: a 2024 systematic review concluded that evidence for NMN improving age-related metabolic outcomes remains equivocal.4 It raises NAD+; whether that translates into a longer healthspan in humans is still being established. Urolithin A. This gut-derived metabolite activates mitophagy — the recycling of damaged mitochondria. It has arguably the cleanest recent trial record of any novel longevity compound. A 2019 first-in-human study in Nature Metabolism established safety and a favourable mitochondrial gene-expression signature,5 and a 2022 randomised trial in Cell Reports Medicine found roughly a 12% improvement in muscle strength over four months in middle-aged adults.6 See our guides to Urolithin A in the UK and the Mitopure review.

Creatine. Long dismissed as merely a gym supplement, creatine is quietly one of the best-evidenced compounds in any longevity stack. It has one of the deepest safety records of any supplement, decades of research behind it, and — crucially for ageing — growing evidence for muscle-preservation and cognitive benefits in older adults. Because sarcopenia (age-related muscle loss) and cognitive decline are two of the defining features of unhealthy ageing, a cheap, safe compound that plausibly addresses both is exactly the kind of unglamorous win this category needs more of. CoQ10. Coenzyme Q10 supports mitochondrial energy production and declines with age. Its standout evidence is the Swedish KiSel-10 trial, in which four years of CoQ10 combined with selenium in elderly people with low selenium status significantly reduced cardiovascular mortality — an effect still detectable twelve years later.7 Note the two caveats: the benefit was seen with selenium and in a selenium-deficient population. Omega-3 and Vitamin D3. The unglamorous foundations, and the ones most people should sort out before spending a penny on anything exotic. Both have extensive trial literature and both address genuine, common shortfalls: oily-fish intake is low across much of the UK population, and vitamin D deficiency is widespread through the British winter — the NHS itself advises considering a vitamin D supplement in the darker months. Omega-3s support cardiovascular and cognitive health; adequate vitamin D underpins bone, muscle and immune function. Neither will appear on a biohacker's "cutting-edge stack" graphic, precisely because neither can be sold at a premium — which tells you a great deal about how the marketing in this category is weighted. If your longevity budget is limited, correcting these two shortfalls is a better first move than any Tier 2 or Tier 3 compound.

Tier 2 — promising, but not yet proven in people

Taurine is the most interesting recent entry. A major 2023 study in Science found that taurine declines with age across species and that restoring it extended healthspan in mice and monkeys and lifespan in mice and worms.8 That generated enormous excitement — but a 2025 analysis in Aging Cell pushed back, arguing the human data do not clearly support taurine deficiency as a driver of ageing in people.9 This is Tier 2 precisely because the human question is genuinely open. Also in this tier: spermidine (an autophagy inducer with encouraging observational data), fisetin (a senolytic under human study), GlyNAC (glycine plus NAC, with promising small trials on glutathione and mitochondrial function), NAC on its own, TMG (a methyl donor often paired with NMN), ergothioneine, astaxanthin, and magnesium — where the threonate versus glycinate distinction matters for how you use it.

Tier 3 — mostly mechanism and mice

Resveratrol is the cautionary tale of the category: spectacular in vitro and in yeast, underwhelming in human outcome trials, with poor bioavailability. Pterostilbene is often promoted as a more bioavailable cousin, but human longevity data are similarly thin. Also here: calcium AKG, modified citrus pectin, C15:0, shilajit, lion's mane, TUDCA, butyrate and Akkermansia. Several are genuinely interesting and may graduate to Tier 2 as trials report — but paying premium prices for Tier 3 compounds today is speculation, not optimisation.

Prescription-only: rapamycin and metformin

Two of the most-discussed molecules in longevity science are not supplements at all. Rapamycin and metformin are prescription medicines under active study (the TAME trial for metformin being the best-known). They require medical supervision and are outside the scope of any self-directed stack. We cover them for completeness, not as a buying recommendation.

The evidence scorecard

Tiers tell you how strong the evidence is, but they don't tell you whether a compound is worth your money. This scorecard adds the two dimensions that actually drive a buying decision: safety and cost. A compound with moderate evidence, excellent safety and a low price may be a far better buy than one with slightly better data at five times the cost.

How to read this: "Overall" weighs evidence strength against safety and cost together. That's why creatine and vitamin D3 score five stars despite being unglamorous — they're well-proven, extremely safe and cheap. Urolithin A has genuinely good trial data but loses a star on cost. Seven common mistakes

Almost every disappointing longevity supplement experience traces back to one of these.

1. Buying underdosed products

A product can contain the right ingredient at a fraction of the studied dose — technically honest labelling, practically useless. Always compare the per-serving dose against the dose used in the trials. Many "longevity blends" contain twelve ingredients at doses too low for any of them to work.

2. Buying without a Certificate of Analysis

3. Starting five supplements at once

If you start everything simultaneously and feel better, you've learned nothing about which compound did it — and you'll keep paying for four things that may be doing nothing. Introduce one at a time.

4. Expecting results in a week

As the timeline above shows, eight to twelve weeks is the minimum fair trial. Abandoning at day ten is the single most common way people waste money in this category.

5. Ignoring sleep and exercise

This is the big one. Supplements are a marginal gain layered on a good foundation. Without adequate sleep and regular movement, you're optimising the last 5% while neglecting the first 80%.

Longevity myths, debunked

Myth: NMN reverses ageing

False. NMN raises NAD+ levels — a real, measurable effect demonstrated in randomised trials. But raising a coenzyme is not reversing ageing, and no human trial has shown NMN extends lifespan. The credible claim is support for cellular energy metabolism and possibly healthspan markers. Anything stronger is marketing.

Myth: supplements can replace exercise

False, and the most costly belief in this category. No compound reproduces the effects of resistance training or Zone 2 cardio on mitochondrial density, muscle mass, insulin sensitivity and cardiovascular function. Supplements complement exercise; they never substitute for it.

Myth: more is better

Myth: expensive means better

Myth: "natural" means safe

Who should not take longevity supplements

Read this before buying anything. Longevity supplements are generally well tolerated in healthy adults, but there are circumstances where they are inappropriate, unstudied, or capable of interacting dangerously with medication. If any of the following applies to you, speak to your GP or pharmacist before taking any supplement in this guide — including the ones we've rated most highly.

Pregnancy and breastfeeding

Almost no longevity compound has been tested for safety in pregnancy or breastfeeding, and absence of evidence is not evidence of safety. NMN, urolithin A, spermidine, fisetin, berberine and most botanical extracts should be avoided. Pregnancy supplementation should follow NHS guidance and your midwife's advice — which does include specific recommendations such as folic acid and vitamin D.

Cancer treatment

This is a genuinely important one. High-dose antioxidants can theoretically interfere with chemotherapy and radiotherapy, both of which work partly through oxidative mechanisms. Some compounds also affect cell growth pathways. Never add supplements during cancer treatment without explicit approval from your oncology team.

Anticoagulants and antiplatelet medication

If you take warfarin, a DOAC, clopidogrel or similar, several supplements can increase bleeding risk — high-dose omega-3, vitamin E, garlic, ginkgo and others. Vitamin K content also affects warfarin control directly. This combination requires pharmacist or GP oversight.

Diabetes medication

Liver disease

The liver metabolises most supplements. Impaired function alters clearance and raises the risk of accumulation. Several botanical extracts, including some green tea and turmeric preparations, have been associated with liver injury in susceptible individuals. Seek specialist advice first.

Kidney disease

Reduced kidney function affects the clearance of many compounds, and some — including creatine, magnesium and potassium-containing products — need particular caution or dose adjustment in chronic kidney disease. Always discuss with your renal team.

Other situations warranting caution

  • Before surgery — many supplements affect bleeding or interact with anaesthesia; most surgeons advise stopping 1–2 weeks beforehand.
  • Children and adolescents — longevity compounds are neither studied nor appropriate in under-18s.
  • Autoimmune conditions or immunosuppressant medication — immune-modulating supplements may interfere.
  • Multiple prescription medications — interaction risk rises with each one; ask your pharmacist to run a check.

The latest longevity research: what's emerging

This field moves quickly, and several compounds have shifted position in the last few years. Here's an honest read on where the emerging science currently stands — including where it has moved against a popular compound.

Urolithin A: the clearest recent success

Urolithin A has moved from obscurity to Tier 1 faster than any compound in recent memory, on the back of a genuinely well-conducted randomised trial showing roughly a 12% improvement in muscle strength alongside reduced inflammatory markers.6 It's expensive, and the primary endpoint in that trial (peak power output) wasn't met — but the direction of travel is positive. Taurine: the cautionary update

This is the most instructive recent story in longevity science. The 2023 Science paper generated enormous excitement by showing taurine declines with age and that restoring it extended healthspan in mice and monkeys.8 But a 2025 analysis in Aging Cell directly challenged the human relevance of that finding.9 Taurine remains cheap, safe and plausible — but anyone selling it as a proven human longevity intervention is ahead of the evidence. Ergothioneine

Sometimes called a "longevity vitamin", ergothioneine is a mushroom-derived antioxidant with a dedicated cellular transporter — unusual, and suggestive of a genuine physiological role. C15:0 (pentadecanoic acid)

Ca-AKG

Calcium alpha-ketoglutarate produced striking results in mouse healthspan studies and has attracted attention from biological-age researchers. Human data remain minimal and it's costly. Worth watching, not yet worth prioritising — see Ca-AKG UK.

GlyNAC

The combination of glycine and N-acetylcysteine to restore glutathione has produced encouraging small trials in older adults, with improvements reported across several ageing markers. Specialised pro-resolving mediators (SPMs)

SPMs are omega-3-derived molecules that actively switch off inflammation rather than merely suppressing it. Given that chronic inflammation is one of the twelve hallmarks of ageing, this is a mechanistically compelling direction — but human longevity data are at a very early stage.

The prescription frontier

No supplement has been proven to extend human lifespan. The strongest evidence supports improvements in healthspan — measurable things like muscle strength, NAD+ levels, mitochondrial markers and cardiovascular risk factors. Lifestyle factors (sleep, exercise, diet, not smoking, blood-pressure control) remain the only interventions with robust human longevity data, and supplements work best as a layer on top of those.
