Somewhere between 40 and 50, most women notice something shift. Recovery takes an extra day. Sleep becomes lighter. Body composition changes though nothing about the diet has. That is not imagination — it is a measurable set of biological changes driven largely by falling oestrogen.

The honest answer is that a small number of nutrients have strong evidence for this specific decade, a second group has promising but early human data, and a third is largely marketing. This guide separates those three tiers.
The short answer
Bone is the priority nobody mentions. SWAN data shows bone loss begins about a year before the final period and decelerates two years after — and roughly 70% of a decade's lumbar spine loss occurs inside that three-year window [5]. The defensible foundation is four things: vitamin D [1], protein at 1.0–1.2 g/kg [15,16], creatine monohydrate at 3–5 g [2,3], and omega-3. Urolithin A has the best trial data of the newer compounds [6,7]. Collagen needs a caveat: a 2025 meta-analysis of 23 trials found benefit overall, but no effect in independently funded studies or high-quality studies, concluding there is currently no clinical evidence supporting collagen for skin ageing [11]. No supplement replaces HRT [18].

Why needs change for women after 40
Ageing is not one process. It is a set of interacting mechanisms — summarised as the hallmarks of ageing — including mitochondrial dysfunction, cellular senescence, loss of proteostasis and altered nutrient sensing. What makes the fifth decade distinctive for women is that a hormonal transition accelerates several of these at once.
Oestrogen decline is the central event
Oestrogen is not only a reproductive hormone. It influences bone turnover, vascular function, lipid handling, collagen synthesis in skin and connective tissue, sleep architecture and how the body partitions fat [19]. When it falls during perimenopause and menopause, several systems change simultaneously — which is why symptoms rarely arrive one at a time.
Bone loss is fastest around the final period — not decades later
This is the single most under-appreciated fact in this age group, and the data is unusually clear.
The Study of Women's Health Across the Nation followed a multi-ethnic cohort through the menopause transition. Bone mineral density loss began about one year before the final menstrual period and decelerated — though did not cease — about two years after it. Over a ten-year observation period, cumulative lumbar spine loss was 10.6%, of which 7.38% occurred inside that three-year "transmenopause" window. Femoral neck loss was 9.1% cumulative, with 5.8% in the same window [5].
Read those numbers again. Roughly seven-tenths of a decade's spine bone loss happens in about three years — a window most women pass through without a bone scan, and without thinking about their skeleton at all. A related review reports faster loss around 2.5% per year at the lumbar spine and 1.8% at the femoral neck during the rapid phase in White women [4]. SWAN also found loss was slower in women with higher BMI and in African American women, and faster in women of Japanese and Chinese ancestry [5].
Muscle, metabolism and energy
Lean mass declines with age in both sexes, but the menopause transition adds an extra push. Less muscle means a lower resting metabolic rate, poorer glucose handling and a weaker buffer against frailty later. Muscle is also the largest amino acid reservoir and a major site of glucose disposal — which makes protecting it arguably the highest-leverage longevity intervention available to a woman in her forties, more so than any capsule.
What supplements can and cannot do
Supplements correct deficiencies, support processes under strain, and occasionally produce measurable functional changes in trials. They do not replace resistance training, protein, sleep or — where clinically appropriate — hormone therapy [18]. If you read nothing else here, read that sentence twice. The foundation: what to get right first
Vitamin D
The NHS advises everyone in the UK consider a daily supplement containing 10 micrograms (400 IU) during autumn and winter, because between roughly October and March UK sunlight is too weak for the skin to synthesise it. Year-round supplementation is advised for people rarely outdoors, who cover their skin, or who have darker skin. The adult upper limit is 100 micrograms (4,000 IU) daily [1].
For women over 40 this matters mainly because vitamin D governs calcium absorption, and calcium absorption is what stands between you and accelerated loss during the transition [4,5]. Many UK women take 1,000–2,000 IU daily, comfortably below the upper limit — but check with your GP first if you have kidney disease, sarcoidosis or take related medication. A vitamin D blood test removes the guesswork.
What about vitamin K2?
K2 (menaquinone-7) is often paired with D3 on the theory it directs calcium into bone rather than arteries. The mechanistic case is reasonable and the safety profile good, but outcome data in healthy postmenopausal women is thinner than the marketing suggests. A sensible optional add-on, not a foundation.
Calcium — food first
Best obtained from food: dairy, fortified plant milks, tinned fish with bones, calcium-set tofu, leafy greens. Routine high-dose supplementation in women who already eat well is not clearly beneficial and has been debated for cardiovascular safety. If dietary intake is genuinely low, a modest supplement alongside vitamin D is reasonable — ideally discussed with your GP, particularly if you have had kidney stones.
Magnesium
Involved in several hundred enzymatic reactions including muscle function, nerve signalling and bone matrix formation, and commonly under-consumed. For women over 40 the most frequently reported subjective benefits concern sleep and muscle cramping. Omega-3 (EPA and DHA)
Oily fish twice weekly is standard advice and most people in the UK do not manage it. Relevant here for cardiovascular risk — which rises after menopause — and for membrane and inflammatory signalling. Look at actual EPA/DHA content rather than total fish oil weight, and choose algal oil if vegan; see vegan longevity supplements.
Protein and creatine: the two most underrated items here
Expert groups including PROT-AGE and ESPEN recommend healthy older adults consume at least 1.0–1.2 g of protein per kilogram of body weight daily — meaningfully above the 0.8 g/kg reference intake — to maintain lean mass and function, with higher intakes during illness [15,16]. For a 65 kg woman that is roughly 65–80 g daily, spread across meals rather than loaded into dinner.
Creatine deserves particular attention for women. Reviews of creatine across the female lifespan report benefits for strength and exercise performance, with favourable effects on bone when combined with resistance training in postmenopausal women [2,3]. B12, folate and iron
B12 absorption declines with age and is lower in vegans, vegetarians and long-term users of metformin or proton pump inhibitors. Folate status interacts with methylation, relevant if you take methyl-consuming compounds — see TMG vs methylfolate.
Iron is the one nutrient where the advice reverses at menopause. While you are still menstruating, iron needs are higher. Once periods stop, routine iron supplementation is usually unnecessary and can be harmful, because the monthly route of loss has gone. Do not supplement iron without a blood test — a ferritin test settles it.
Longevity-specific supplements, tier by tier
NMN and NAD⁺ precursors
NAD⁺ is a coenzyme central to energy metabolism and DNA repair, and cellular levels decline with age. NMN and nicotinamide riboside are precursors used to raise it. This is the most popular category in longevity, and worth being precise about.
A 2024 systematic review of ten randomised controlled trials covering 437 participants (mean age 58, doses 150–1,200 mg/day, follow-up 4–12 weeks) examined NMN and physical performance and reported a reassuring safety profile [8]. But a separate 2024 meta-analysis of eight RCTs in 342 middle-aged and older adults found no significant benefit at 250–2,000 mg/day on fasting glucose, insulin, HbA1c, insulin resistance or lipid profile over 14 days to 12 weeks [9].
The fair summary: NMN reliably raises NAD⁺ markers and appears well tolerated, but short trials in relatively healthy adults have not demonstrated broad metabolic benefit. That is reason for measured expectations, not dismissal — see is NMN a scam?
Regulatory status in the UK — rarely explained properly
Nicotinamide riboside chloride is an authorised novel food in Great Britain, permitted in food supplements at up to 300 mg/day for the general adult population and 230 mg/day for pregnant and breastfeeding women [13]. β-nicotinamide mononucleotide is the subject of a regulated product application with the Food Standards Agency rather than a completed authorisation [14]. EFSA has separately published a safety opinion on β-NMN for use in food supplements as a source of niacin, in adults excluding pregnant and lactating women [17].

Urolithin A
A gut-microbiome metabolite of ellagitannins found in pomegranates. Only some people produce it efficiently from food, which is the rationale for supplementing directly. It works by stimulating mitophagy — the recycling of damaged mitochondria.
The human data here is unusually good for a longevity ingredient. A four-month randomised, placebo-controlled trial in 88 untrained, overweight middle-aged adults reported roughly 12% improvements in muscle strength at both 500 mg and 1,000 mg, along with reduced plasma acylcarnitines and C-reactive protein — though it missed its primary endpoint of peak power output [6]. A separate randomised trial in adults aged 65–90 using 1,000 mg daily for four months found improvements in muscle endurance and mitochondrial biomarkers, while also missing its primary endpoint of six-minute walk distance [7].

Collagen peptides — read this section before buying
Skin collagen declines sharply around menopause, which is why this category is marketed so heavily to women over 40. We sell collagen, and the evidence has moved in a direction that customers deserve to hear.
A meta-analysis of 19 randomised, double-blind, controlled trials involving 1,125 participants (95% women) found hydrolysed collagen for around 90 days improved skin hydration and elasticity and reduced wrinkles versus placebo [10]. That was the settled position until recently.
The 2025 analysis that changed the picture
Myung and Park pooled 23 RCTs in 1,474 participants for The American Journal of Medicine. Overall, collagen significantly improved hydration, elasticity and wrinkles. Then they split the trials by who paid for them.
Studies not funded by pharmaceutical or supplement companies showed no effect on hydration, elasticity or wrinkles — none. Industry-funded studies showed significant effects across all three. When separated by study quality, high-quality trials showed no significant effect. The authors' conclusion: there is currently no clinical evidence to support using collagen supplements to prevent or treat skin ageing [11]. They declared no funding and no conflicts of interest.
In fairness, industry has pushed back. Critics note that 21 of the 23 trials had some form of industry involvement, making the funded/non-funded split blurrier than presented, and that several regulators internationally have accepted skin claims for collagen peptides [12]. That is a legitimate methodological argument — but the burden has shifted, and anyone selling collagen for skin should say so.

CoQ10 and ubiquinol
Required for mitochondrial electron transport, with endogenous levels falling with age. Most clearly relevant for women taking statins, which reduce CoQ10 synthesis. Ubiquinol is the reduced form and generally better absorbed, at higher cost — see CoQ10 vs ubiquinol and PQQ vs CoQ10.
Spermidine, taurine, Ca-AKG and the early-stage group

Building a stack by life stage
Age 40–45: early perimenopause
Cycles may still be regular but sleep and recovery often change first. Priorities: vitamin D, adequate protein, creatine, omega-3, and magnesium if sleep is disrupted. This is also the most sensible point to start an NAD⁺ precursor if you intend to use one, since the rationale is preventive rather than corrective.
Age 45–55: the menopause transition
The highest-stakes window, because it overlaps the period of fastest bone loss [5]. Priorities: everything above, plus deliberate attention to calcium from food, resistance training at least twice weekly, and a conversation with your GP about whether HRT is appropriate for you [18]. Collagen and urolithin A are reasonable additions if skin and energy are your primary concerns — with the collagen caveat above.
Age 55 and beyond
Priorities shift toward preserving function: protein and creatine become more important, not less; iron supplementation should generally stop unless blood tests indicate otherwise; B12 status is worth checking.
A minimal, defensible stack

Timing, absorption and expectations
Timing matters less than consistency, but a few points are worth getting right. Vitamin D, omega-3 and CoQ10 are fat-soluble and absorb better with a meal containing fat. Creatine can be taken any time. Magnesium is usually taken in the evening if sleep is the goal. NAD⁺ precursors are commonly taken in the morning, though the human evidence for timing specifically is thin.

Safety, interactions and who should be cautious
This section matters more than the product recommendations. Supplements are not risk-free, and "natural" is not a safety category.
If you take HRT
Most supplements here have no known interaction with hormone therapy, but HRT is a prescribed medicine and any new regimen should be mentioned to the clinician managing it. Supplements are not an alternative to HRT for managing menopausal symptoms, and framing them that way does women a disservice. If you take thyroid medication
Levothyroxine absorption is reduced by calcium, iron and some mineral supplements. Separate by at least four hours and follow your prescriber's instructions. Thyroid dysfunction can also mimic perimenopausal symptoms — a thyroid blood test is worth having before assuming hormones are the cause.
If you have a hormone-sensitive condition or a history of breast cancer
Be cautious with any supplement marketed as "oestrogenic", "phytoestrogen-rich" or "hormone balancing". Discuss anything in this category with your oncology or gynaecology team before starting.
If you are pregnant, breastfeeding or trying to conceive
Several longevity compounds — including NMN — have not been assessed for pregnancy or lactation, and regulatory safety opinions specifically exclude these groups [17]. Do not use them. This remains relevant in perimenopause, when pregnancy is still possible.
If you take anticoagulants, blood pressure or diabetes medication

When to see your GP rather than reach for a supplement
Book an appointment for unexplained fatigue that is not improving, heavy or irregular bleeding, any bleeding after the menopause, unexplained weight loss, a new breast lump or breast change, new breathlessness or palpitations, a fracture from a minor fall, or persistent low mood. These need investigation, not supplementation. How to judge quality
- Third-party testing — an independent laboratory verifying identity, purity and contaminants. See third-party tested supplements.

What a sensible stack costs
A foundation stack — vitamin D, omega-3, creatine and magnesium — can realistically run for £15–£25 per month. Adding an NAD⁺ precursor typically takes it to £40–£70. Adding urolithin A at clinical doses is the single biggest jump, often £50–£90 alone. A full "everything" stack can exceed £150 monthly, which is rarely a good use of money relative to food quality and a gym membership.

Supplements that don't earn their place
Being useful means being willing to say what to skip. Resveratrol has poor oral bioavailability and disappointing human results relative to the hype — see does resveratrol work? and resveratrol vs pterostilbene. Low-dose "longevity blends" listing twenty ingredients at sub-clinical amounts are almost always worse value than two or three ingredients at proper doses.
