Zolmitriptan

• Your headache symptoms are getting worse or more frequent and you have not seen your GP
  
• You suffer from basilar, hemiplegic or ophthalmoplegic migraine

• Any of the ‘triptan’ family of medicines eg. sumatriptan (Imitrex, Imigran, Cinie, Illument, Migriptan), rizatriptan (Maxalt), naratriptan (Amerge, Naramig), zolmitriptan (Zomig), eletriptan (Relpax), almotriptan (Axert, Almogran), frovatriptan (Frova, Migard, Frovamig), and avitriptan
• Sulphonamides

• Previous cerebrovascular accident or transient ischaemic attack (TIA)
• Peripheral vascular disease
• Predisposition to coronary artery disease
• Ischaemic heart disease
• Risk factors for heart disease such as being a heavy smoker or using nicotine replacement therapy products
• Previous myocardial infarction
• History of or current Stroke or mini stroke
• Coronary vasospasm (Prinzmetal's angina)
• Heart failure
• Wolff-Parkinson-White syndrome or arrhythmias associated with accessory cardiac conduction pathways.
• Moderate, severe or uncontrolled hypertension
• Hemiplegic, basilar or opthalmoplegic migraine
• History of seizures
• Liver or kidney problems
• Galactose intolerance, Lapp Lactase deficiency or glucose-galactose malabsorption
• Recent rash associated with the headache

• Antidepressants (TCA’s) eg. Clomipramine, Imipramine, Lofepramine, Nortriptyline, Amitriptyline, Dosulepin, Trimipramine
• Antidepressants (SSRI’s) eg. Citalopram, Escitalopram, Fluoxetine, Fluvoxamine, Paroxetine, Sertraline etc.
• Antidepressants (SNRI’s) eg. Duloxetine (Cymbalta, Yentreve), Duloxetine (Cymbalta, Yentreve)
• Antidepressants (MAOI’s) eg. Moclobemide, Isocarboxazid, Phenelzine, Tranylcypromine, St. John’s Wort etc.
• Other 'triptan' medicines used to treat migraine (eg sumatriptan (Imigran, Migriptan), rizatriptan (Maxalt), naratriptan (Naramig), zolmitriptan (Zomig), eletriptan (Relpax), almotriptan (Almogran), frovatriptan (Frova, Migard, Frovamig))
• Ergotamine or other ergot medicine
• Medicines for high blood pressure
• The combined oral contraceptive pill (not the progesterone only ‘mini’ pill)
• Propranolol
• Pizotifen
• Linezolid
• Antibiotics including but not limited to Clarithromycin, Erythromycin or Azithromycin
• Antifungal medication including by not limited to Ketoconazole, Itrazonazole or Fluconazole
• Antiviral medication including by not limited to Indinavir, Nelfinavir or Ritonavir
• Sibutramine
• Reboxetine
• Tryptophan
• Dexamfetamine
• Fentanyl, Tramadol or Tapentadol

• Go to hospital A&E if after a dose of triptan you experience chest pain or a feeling of pressure in the chest or throat that lasts for more than a few minutes (this could be the symptoms of a heart attack)
• Discontinue any further treatment with a triptan and seek immediate medical advice.
• You will read the patient information leaflet supplied with the medication
• You will contact us and inform your GP of your medication if you experience any side effects of treatment, if you start new medication or if your medical conditions change during treatment
• The treatment is solely for your own use
• You have answered all the above questions accurately and truthfully. You understand our prescribers take your answers in good faith and base their prescribing decisions accordingly, and that incorrect information can be hazardous to your health
• You understand that whilst decisions relating to your treatment are made jointly between you and the prescriber, the final decision to issue a prescription will always be with the prescriber.

• Prolonged use of any type of painkiller for headaches can make them worse, do not take more than once a week without seeking medical advice
• Drowsiness or dizzyness may occur as a result of migraine or its treatment with a triptan. If affected do not drive or operate machinery
• Triptans should not be used prophylactically, rather at, or as soon as possible, after the onset of a migraine

• You will seek medical attention if you experience any unusual side effects
• The treatment is solely for your own use
• You will read the patient information leaflet supplied with your medication

• do you have migraine attacks on more than 8 days a month?
• do you have two or more attacks per month that cause disability lasting for 3 days or more?
• have you ever had a migraine that lasted more than 72 hours?
• do you use triptans or painkillers for your migraines on two or more days a week on a regular basis?
• Your headaches started after the age of 40 and you have not seen your GP about them
